Bankers Health Advantage - Privacy Statement

This plan is NOT insurance.

The plan provides discounts at certain health care providers of medical services. The plan does not make payments directly to the providers of medical services.

The plan member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount plan organization. The range of discounts for services offered will vary depending on the type of provider and service. Discounts on hospital services are not available in Maryland. This is not a Medicare prescription drug plan.

This plan is administered by Coverdell & Company, Inc., a discount medical plan organization at 8420 W. Bryn Mawr, Suite 700, Chicago, IL 60631, 1-800-308-0374.

You have the right to cancel this plan within 30 days of the effective date for a full refund of fees paid.

Customer privacy is important to us*. Our members have chosen to do business with us; and we recognize our obligation to keep the information we learn about them secure and confidential. The following is an outline of the principles of our commitment:

• We intend to collect only nonpublic personal information needed to deliver superior products and services. This includes information we need to process applications, administer accounts, and notify customers of products or services that we believe would be of interest to them.

• We will require that only those employees with appropriate authority have access to this data. They will use and disclose this information for business purposes only.

• We will not share personally identifiable medical information about our customers with affiliated or non-affiliated companies, unless the information is needed to underwrite or administer your account.

• Whenever we employ non-affiliated companies or individuals to perform functions for us, we will give them access only to the information needed to perform those functions and will not allow them to use it for any other purpose.

• We will require these non-affiliated companies to abide by our privacy standards and retain the data only long enough to perform the stated service except as required by law.

• In response to a court order, subpoena, search warrant, law or regulation, we may be legally compelled to release your personal information. If that happens, we will notify you, unless doing so would violate the law or court order.

• We may analyze utilization information in the aggregate and license this aggregate data (with no link to your identity) to third parties for research purposes.

• We may also provide nonpublic personal member information to non-affiliated
companies that offer, on our behalf, products or services that may benefit our
members. If we do so, however, we will not:
- Release a member’s account number
- Release the information for an unspecified use
- Share a member’s information if we have been asked not to do so

We will maintain the same privacy standards for current and former members; and we will fully comply with all federal and state law and regulatory requirements that protect consumer privacy.
